Effects and efficacy:
It is mainly used in combination with other anti-tuberculosis drugs for the initial treatment of various tuberculosis cases caused by Mycobacterium tuberculosis, or other sensitive mycobacterial infections. It can be used alone to treat tularemia, or in combination with other antibacterial drugs for the treatment of plague, inguinal granuloma, brucellosis, rat-bite fever, etc. It can also be combined with penicillin or ampicillin to treat endocarditis caused by viridans streptococci or enterococci.
Usage and dosage:
Common dosage for adults with tuberculosis: intramuscular injection, 0.5g every 12 hours (calculated as streptomycin, the same below), or 0.75g once a day, combined with other anti-tuberculosis drugs; if intermittent therapy is used, that is, 2 to 3 times a week, 1g each time; intramuscular injection for elderly patients, 0.5 to 0.75g once a day. Bacterial (Streptococcus aureus) endocarditis: intramuscular injection, 1g every 12 hours, combined with penicillin, for 1 week, followed by 0.5g every 12 hours for 1 week; patients over 60 years old should reduce to 0.5g every 12 hours for 2 weeks. Enterococcal endocarditis: intramuscular injection, combined with penicillin, 1g every 12 hours for 2 weeks, followed by 0.5g every 12 hours for 4 weeks. Common dosage for children to treat tuberculosis: 20mg/kg per day, once a day, the maximum daily dose does not exceed 1g, combined with other anti-tuberculosis drugs. Other infections: 15~25mg/kg per day, divided into 2 doses; Patients with renal impairment: The normal dose of streptomycin for patients with normal renal function is once a day, 15mg/kg intramuscular injection. If the creatinine clearance is 50~90ml/min, 50% of the normal dose is given every 24 hours; if the creatinine clearance is 10~50ml/min, 50% of the normal dose is given every 24~72 hours. Adverse reactions:
The main side effect of streptomycin is damage to the eighth cranial nerve, the auditory nerve, causing tinnitus, dizziness, loss of coordination during exercise, tinnitus, and a feeling of fullness in the ears. In severe cases, hearing loss or even deafness may occur. If pregnant women use streptomycin, it can be transmitted to the fetus through the placenta, causing congenital deafness. A small number of patients may still experience ototoxic symptoms such as hearing loss, tinnitus, and fullness in the ears after stopping the drug, which should be paid attention to. Damage to the kidneys can be seen, and nephrotoxic symptoms such as hematuria, decreased urination frequency or decreased urine volume, loss of appetite, thirst, etc., and a few may produce increased urea nitrogen and creatinine in the blood. Therefore, it is not recommended for patients with renal insufficiency. If elderly patients must use it, they should also be cautious or use it in reduced doses. When the vestibular function is affected, there may be symptoms such as unsteady gait and dizziness. Some patients may experience symptoms of peripheral neuritis such as numbness of the face or limbs, and tingling. Occasionally, vision loss (optic neuritis), neuromuscular blockade symptoms such as drowsiness, weakness, and difficulty breathing may occur.
