Effects and efficacy:
Roxadustat is used to treat anemia caused by chronic kidney disease (CKD) in patients undergoing dialysis.
Dosage and administration:
Roxadustat is used orally. The specific dosage should be prescribed by the doctor. Initial treatment should be carried out under the supervision of professional medical personnel. Select the starting dose according to weight: 100 mg (45~60kg) or 120 mg (≥60kg) per time for dialysis patients, orally three times a week (TIW). The drug can be taken on an empty stomach or after eating; patients undergoing hemodialysis and peritoneal dialysis can take this product at any time before and after dialysis. During the initial treatment phase, it is recommended to monitor the hemoglobin (Hb) level once every 2 weeks until it stabilizes; then monitor Hb once every 4 weeks. The dose of roxadustat should be adjusted according to the Hb level so that the Hb level reaches and is maintained between 100~120g/L and the need for blood transfusion is minimized. It is recommended to adjust the dose every 4 weeks based on the patient’s current Hb level and the change in Hb in the past 4 weeks. Hb change in the past 4 weeks: <-10g/L: When adjusting the dose, Hb <120g/L, the dose should be increased according to the preset dose steps; when adjusting the dose, Hb ≥120g/L, <130g/L, no dose change. Hb change in the past 4 weeks: ≥-10g/L, ≤10g/L: When adjusting the dose, Hb <105g/L, the dose should be increased according to the preset dose steps; when adjusting the dose, Hb ≥105g/L, <120g/L, no dose change; when adjusting the dose, Hb ≥120g/L, <130g/L, the dose should be reduced according to the preset dose steps. Hb change in the past 4 weeks: >10g/L: When adjusting the dose, Hb <105g/L, no dose change; when adjusting the dose, Hb ≥105g/L, <130g/L, the dose should be reduced according to the preset dose steps. Regardless of the changes in Hb over the past 4 weeks, when adjusting the dose, if Hb ≥ 130g/L, the drug should be suspended and Hb should be monitored until Hb < 120g/L, then the dose should be reduced by one step and the drug should be resumed. Dose increases and decreases are increased and reduced according to the preset dose steps, which are 20, 40, 50, 70, 100, 120, 150 and 200mg. Dose adjustment when Hb is too high and too fast: If the patient’s Hb increases by &gt; 20g/L within 2 weeks and the Hb value is &gt; 90g/L, the dose should be reduced by one step. When Hb rises too fast, it is recommended to reduce the dose only once within 4 weeks.
