Effects and efficacy:
Ganciclovir tablets, ganciclovir capsules, ganciclovir dispersible tablets: used for patients with cytomegalovirus infection caused by immune damage. Maintenance treatment of cytomegalovirus retinitis caused by immune damage (including AIDS patients). Prevention of cytomegalovirus infection that may occur in organ transplant recipients. Prevention of cytomegalovirus infection in patients with advanced HIV infection. Ganciclovir eye gel, ganciclovir eye drops: used for herpes simplex virus keratitis. Ganciclovir for injection, ganciclovir injection: This product is suitable for the prevention of cytomegalovirus disease that may occur in organ transplant recipients at risk of cytomegalovirus infection. Treatment of cytomegalovirus retinitis that may occur in immunocompromised patients (including AIDS patients).
Usage and dosage:
Ganciclovir tablets, ganciclovir capsules, and ganciclovir dispersible tablets can be taken directly orally/swallowed, and dispersible tablets can be put into about 100ml of water and shaken to disperse before oral administration. Under normal renal function: Maintenance treatment of cytomegalovirus CMV retinitis: After induction therapy, the recommended maintenance dose is 1000 mg each time, 3 times a day, taken with food. It can also be taken at a non-sleeping time, 500 mg each time, once every 3 hours, 6 times a day, taken with food. If CMV retinitis develops during maintenance treatment, induction therapy should be repeated. Prevention of CMV disease in patients with advanced human immunodeficiency virus HIV infection: The preventive dose is 1000 mg each time, 3 times a day, taken with food. Prevention of CMV disease in organ transplant recipients: The preventive dose is 1000 mg each time, 3 times a day, taken with food. The course of medication is determined according to the duration and degree of immunosuppression. If the patient’s renal function is impaired, the dosage should be adjusted as appropriate according to the creatinine clearance rate, as follows: If the creatinine clearance rate is ≥70ml/min, the dose of ganciclovir capsules is 1000 mg once, 3 times a day; or 500 mg once, 6 times a day, taken once every 3 hours. Creatinine clearance is 50~69ml/min, and the dosage of ganciclovir capsules is 1500mg or 500mg per day, 3 times a day. Creatinine clearance is 25~49ml/min, 1000mg per day or 500mg once, 2 times a day. Creatinine clearance is 10~24ml/min, 500mg per day. Creatinine clearance is <10ml/min, following hemodialysis, 3 times a week, 500mg once. Ganciclovir ophthalmic gel usage: external use, apply to the conjunctival sac. Dosage: about 8mm at a time, 4 times a day, 3 weeks of treatment. Ganciclovir eye drops are dripped into the eyelids, 2 drops at a time, once every 2 hours, 7~8 times a day. Ganciclovir eye ointment is applied to the eyelids, 4~6 times a day, 5~6mm each time (containing about 0.25~0.3mg of ganciclovir). Ganciclovir for injection induction phase: intravenous drip of 5 mg/kg per body weight, once every 12 hours, each drip for more than 1 hour, the course of treatment is 14 to 21 days, and the dose should be reduced for patients with renal impairment. When the creatinine clearance is 50-69 ml/min, intravenous drip of 2.5 mg/kg every 12 hours; when the creatinine clearance is 25-49 ml/min, intravenous drip of 2.5 mg/kg every 24 hours; when the creatinine clearance is 10-24 ml/min, intravenous drip of 1.25 mg/kg every 24 hours; when the creatinine clearance is <10 ml/min, the drug is given 3 times a week, 1.25 mg/kg each time after hemodialysis. Maintenance phase: intravenous drip of 5 mg/kg per body weight, once a day, for more than 1 hour. For patients with impaired renal function, the dosage is adjusted according to the creatinine clearance: when the creatinine clearance is 50-69 ml/min, 2.5 mg/kg is given by intravenous drip every 24 hours; when the creatinine clearance is 25-49 ml/min, 1.25 mg/kg is given by intravenous drip every 24 hours; when the creatinine clearance is 10-24 ml/min, 0.625 mg/kg is given by intravenous drip every 24 hours; when the creatinine clearance is less than 10 ml/min, the drug is given three times a week, 0.625 mg/kg each time after hemodialysis. Preventive medication: intravenous drip of 5 mg/kg per body weight, the drip time is at least 1 hour, once every 12 hours, for 7-14 consecutive days; followed by 5 mg/kg, once a day, for a total of 7 days. When this product is intravenously infused, the preparation method is as follows: first determine the dosage according to the patient's weight, dissolve it with an appropriate amount of injection water or sodium chloride injection, and then inject it into 100ml of sodium chloride injection, 5% glucose injection, compound sodium chloride injection or compound sodium lactate injection. The concentration of the infusion solution shall not exceed 10mg/ml. Ganciclovir injection is for patients with normal renal function. Prevention of cytomegalovirus disease in organ transplant recipients: initial dose: 5mg/kg, once every 12 hours, intravenous drip at a constant rate, each drip time is more than 1 hour, and it is used continuously for 7 to 14 days. Maintenance dose: 5 mg/kg, once a day, 7 days/week, constant rate intravenous drip, each drip time is more than 1 hour; or 6 mg/kg, once a day, 5 days/week, constant rate intravenous drip, each drip time is more than 1 hour. Special medication instructions. For patients with renal insufficiency, refer to the following table for the recommended dose and medication interval adjustment: Creatinine clearance ≥ 70 ml/min, initial dose 5.0 mg/kg, medication interval 12 hours, maintenance dose 5.0 mg/kg, medication interval 24 hours. Creatinine clearance 50~69 ml/min, initial dose 2.5 mg/kg, medication interval 12 hours, maintenance dose 2.5 mg/kg, medication interval 24 hours. Creatinine clearance 25~49 ml/min, initial dose 2.5 mg/kg, medication interval 24 hours, maintenance dose 1.25 mg/kg, medication interval 24 hours. Creatinine clearance rate 10~24ml/min, initial dose 1.25mg/kg, medication interval 24 hours, maintenance dose 0.625mg/kg, medication interval 24 hours. Creatinine clearance rate <10ml/min, initial dose 1.25mg/kg, medication interval 24 hours, maintenance dose 0.625mg/kg, 3 times a week, after hemodialysis. Adverse reactions: This product can cause granulocytopenia, neutropenia and thrombocytopenia. Rare: headache, dizziness, dyspnea, nausea, vomiting, abdominal pain, diarrhea, anorexia, gastrointestinal bleeding, arrhythmia, increased or decreased blood pressure, chills, hematuria, increased blood urea ammonia, hair loss, pruritus, urticaria, decreased blood sugar, edema, general discomfort, increased creatinine, eosinophilia, etc. AIDS patients with cytomegalovirus retinitis may experience retinal detachment.
