bisoprolol

Effects and efficacy:
Used to treat hypertension, coronary heart disease (angina pectoris), and chronic stable heart failure. If used to treat chronic stable heart failure, before using this product, it is necessary to receive angiotensin-converting enzyme inhibitors, diuretics, and selective use of cardiac glycosides as prescribed by a doctor.

Usage and dosage:
Oral medication. This product should be taken in the morning (it can be taken with meals), and the whole tablet should be taken with water, and it should not be chewed. Please follow the doctor’s advice for details. The treatment of hypertension and angina pectoris is usually once a day, 5 mg of bisoprolol fumarate each time. Patients with mild hypertension can start treatment with 2.5 mg. If the effect is not obvious, the dose can be increased to once a day, 10 mg each time. Please follow the doctor’s advice for details. The dosage of this product should be adjusted according to personal conditions, and special attention should be paid to the pulse and treatment effect during medication. This product is suitable for long-term use. The dosage of this drug cannot be changed without a doctor’s advice, and it is not advisable to stop taking the drug. If you need to stop taking the drug, you should stop it gradually, not suddenly, and patients with ischemic heart disease should pay special attention. Treatment of chronic stable heart failure Patients who have not had acute heart failure within 6 weeks and whose basic treatment has not changed in the past 2 weeks should first receive appropriate doses of angiotensin-converting enzyme inhibitors (other vasodilators can be used if not tolerated), diuretics, and selective use of cardiac glycosides before receiving bisoprolol treatment. Start with a low dose and gradually increase the dose according to the following scheme: 1.25 mg once a day for 1 week; then, depending on the patient’s tolerance, increase the dose by 1.25 mg every week, that is, adjust to 2.5 mg, 3.75 mg, and 5 mg once a day; 5 mg once a day for 4 weeks, and if well tolerated, increase to 7.5 mg once a day for 4 weeks; the maximum tolerated dose that can be achieved is used as the maintenance dose, not exceeding 10 mg (once a day). After the first 1.25 mg treatment, the patient should be observed for about 4 hours (especially blood pressure, heart rate, conduction disorders, signs of worsening heart failure). If adverse reactions occur, the maximum dose should be avoided and, if necessary, the dose should be gradually reduced based on the existing dose. If necessary, treatment can be interrupted and the drug can be used again when appropriate. During the dose escalation period, if heart failure worsens or intolerance occurs, it is recommended to reduce the dose of bisoprolol first, or stop the drug immediately if necessary (if severe hypotension, worsening of heart failure accompanied by acute pulmonary edema, cardiogenic shock, symptomatic bradycardia or atrioventricular block occurs). Note: This drug should be used for a long time to treat chronic stable heart failure. However, it should not be stopped suddenly to avoid temporary worsening of heart failure. If the drug needs to be stopped, the dose should be gradually reduced by half every week. Please follow the doctor’s advice for details. Patients with hepatic and renal insufficiency usually do not need to adjust the dose. For patients with advanced renal failure (creatinine clearance < 20ml/min) and severe liver function abnormalities, the daily dose should not exceed 10mg. There is little experience with the use of bisoprolol in patients with renal dialysis, but there is no evidence that the dose should be adjusted in such patients. There is no pharmacokinetic data on the use of bisoprolol in patients with chronic heart failure and hepatic and renal insufficiency. The dose increase of such patients should be particularly cautious.
